Payment must be made within 24 hours of the submission of this form. DownDog Yoga will only reserve your interest till that time. After 24 hours of the submission of this membership form, if no payment is received by DownDog Yoga, you will be required to complete a new membership form and confirmation of requested membership/workshop/private classes will be subject to availability.
Waiver/Disclaimer :
I/We understand that it is my/our responsibility to consult with a physician prior to participating in any physical fitness program, including yoga, and to inform the instructor of any medical conditions or physical limitations.
I agree to assume full responsibility for any risks, injuries or damages, known or unknown, that

I further understand that failure to comply with the stated Terms and Conditions of Use may result
in the revocation of my participation in DownDog Yoga India activities.
